1. Requests You May Make

2. Information to Include

Your full name and sufficient contact information for a response.
Your relationship with the clinic, such as website visitor, prospective patient or patient.
A clear description of your request and, where possible, the relevant date or communication channel.
Your preferred response method.

Please do not include unnecessary medical information. Do not e-mail a passport, national identity card or similar document unless the clinic has first identified a necessary and secure verification method.

The clinic may request proportionate additional information to verify that the applicant is the data subject or an authorised representative. Applications are handled within the period and procedure required by applicable law.
